Contraindications

Interaction with other drugs

До начала терапии, через 6 недель и 12 недель после начала приема препарата Тулип® или после увеличения его дозы, необходимо контролировать показатели функции печени (АСТ, АЛТ). Функцию печени следует контролировать также при появлении клинических признаков поражения печени. В случае повышения активности АСТ и АЛТ, их активность следует контролировать до тех пор, пока она не нормализуется. Применяя препарат Тулип® в комбинации с фибратами, эритромицином, иммунодепрессантами, азольными противогрибковыми препаратами или никотиновой кислотой в липидснижающих дозах (более 1 г/сут), необходимо взвесить ожидаемую пользу и риск от лечения препаратом Тулип®.
Очень редко сообщалось о случаях иммуноопосредованной некротизирующей миопатии во время или после лечения статинами, включая аторвастатин. Интерстициальная болезнь легких

Dosing and Administration
Перед применением препарата Тулип® пациенту необходимо рекомендовать стандартную гипохолестеринемическую диету, которую он должен продолжать соблюдать в течение всего периода терапии.
Рекомендуется применять препарат Тулип® внутрь независимо от времени приема пищи. Доза препарата Тулип® варьируется в пределах от 10 мг до 80 мг в сутки, и подбирается с учетом исходных концентрации холестерина-ЛПНП, цели терапии и индивидуального терапевтического ответа на проводимую терапию.

Первичная (гетерозиготная наследственная и полигенная) гиперхолестеринемия (тип IIa) и смешанная гиперлипидемия (тип IIb)
Начальная доза 10 мг препарата Тулип® 1 раз в сутки (возможно применение препарата аторвастатин в лекарственной форме: таблетки по 10 и 20 мг). При необходимости возможно постепенное увеличение дозы до 80 мг (2 таблетки по 40 мг) в зависимости от реакции пациента с интервалом 2 – 4 недели, поскольку терапевтический эффект отмечается через 2 недели, а максимальный терапевтический эффект – через 4 недели. Гомозиготная наследственная гиперхолестеринемия

У пациентов с нарушением функции печени замедляется выведение аторвастатина из организма, поэтому рекомендуется применять его с осторожностью при регулярном контроле активности «печеночных» трансаминаз: аспартатаминотрансферазы (АСТ) и аланинаминотрансферазы (АЛТ). Если наблюдаемое повышение активности АСТ или АЛТ более чем в 3 раза по сравнению с верхней границей нормы (ВГН) сохраняется, рекомендуется снижение дозы или отмена препарата Тулип®.
