Частота тяжелых нарушений функции почек повышается при приеме 40 мг розувастатина. Рекомендуется контролировать показатели функции почек во время терапии розувастатином.
При применении препарата Сувардио® во всех дозах и, в особенности при приеме препарата в дозе, превышающей 20 мг, были выявлены миалгия, миопатия и, в редких случаях, рабдомиолиз. Очень редко возникал рабдомиолиз при одновременном приеме эзетимиба и ингибиторов ГМГ-КоА-редуктазы.
В этом случае нельзя исключить фармакологическое взаимодействие препаратов, поэтому совместно препарат Сувардио® и эзетимиб следует применять с осторожностью.
Частота случаев рабдомиолиза при приеме 40 мг препарата Сувардио® увеличивается.
Определение активности КФК не следует проводить после интенсивных физических нагрузок или при наличии других возможных причин увеличения активности КФК, что может привести к неверной интерпретации полученных результатов. В случае если активность КФК до начала терапии существенно повышена (в 5 раз выше, чем ВГН), через 5-7 дней следует провести повторное измерение. Не следует начинать терапию препаратом Сувардио®, если повторный тест подтверждает исходную активность КФК (выше более чем в 5 раз по сравнению с ВГН).
Розувастатин, как и другие ингибиторы ГМГ-КоА-редуктазы, следует с особой осторожностью назначать пациентам с имеющимися факторами риска миопатии/рабдомиолиза. К таким факторам относятся: почечная недостаточность; гипотиреоз (для дозы 40 мг); миопатия в анамнезе (в т.ч. наследственная) (для дозы 40 мг); наличие в анамнезе миотоксичности на фоне приема других ингибиторов ГМГ-КоА- редуктазы или фибратов (для дозы 40 мг); злоупотребление алкоголем (для дозы 40 мг); возраст старше 65 лет; состояния, сопровождающиеся увеличением плазменной концентрации розувастатина (для дозы 40 мг); одновременный прием фибратов (для дозы 40 мг).
У таких пациентов следует оценить соотношение риска и возможной пользы терапии и осуществлять клиническое наблюдение на протяжении всего курса терапии.
Рекомендуется проинформировать пациентов о необходимости незамедлительно сообщать врачу о случаях неожиданного появления мышечных болей, мышечной слабости или спазмах, особенно в сочетании с недомоганием или лихорадкой!
У таких пациентов следует обязательно осуществлять контроль активности КФК. Лечение следует прекратить, если активность КФК более чем в 5 раз превышает ВГН или если мышечные симптомы резко выражены и вызывают ежедневный дискомфорт в течение всего дня (даже если активность КФК в 5 раз меньше ВГН). Если симптомы исчезают, и активность КФК возвращается к норме, следует рассмотреть вопрос о повторном назначении препарата или назначении альтернативного ингибитора ГМГ-КоА-редуктазы в меньших дозах при тщательном наблюдении за пациентом. Регулярный контроль активности КФК у пациентов при отсутствии симптомов рабдомиолиза нецелесообразен.
Не отмечено признаков увеличения нежелательных явлений со стороны скелетной мускулатуры при приеме препарата Сувардио® и сопутствующей терапии. Однако увеличение числа случаев миозита и миопатии было выявлено у пациентов, принимавших другие ингибиторы ГМГ-КоА-редуктазы совместно с производными фибриновой кислоты, включая гемфиброзил, циклоспорин, никотиновую кислоту в липидснижающих дозах (более 1 г/сут), азольные противогрибковые препараты, ингибиторы протеаз и антибиотики группы макролидов. Гемфиброзил увеличивает риск возникновения миопатии при сочетанном назначении с некоторыми ингибиторами ГМГ-КоА-редуктазы. Поэтому одновременный прием розувастатина и гемфиброзила не рекомендован. Необходимо тщательно оценить соотношение риска и возможной пользы при совместном применении розувастатина с фибратами или никотиновой кислотой в липидснижающих дозах (более 1 г/сут). Противопоказан одновременный прием розувастатина в дозе 40 мг и фибратов.
Препарат Сувардио® не следует назначать пациентам с острыми, тяжелыми заболеваниями, с подозрением на миопатию или с возможным развитием вторичной почечной недостаточности (например, сепсис, артериальная гипертензия, хирургическое вмешательство, травма, метаболический синдром, сахарный диабет, судороги, эндокринные нарушения, водно-электролитные нарушения).
Через 2-4 недели после начала лечения и/или при повышении дозы препарата необходим контроль показателей липидного обмена (при необходимости требуется коррекция дозы).
Как и другие ингибиторы ГМГ-КоА-редуктазы, розувастатин следует с особой осторожностью назначать пациентам, злоупотребляющим алкоголем или имеющим в анамнезе заболевания печени.
Рекомендуется проводить определение показателей функции печени до и через 3 месяца после начала лечения. Если активность «печеночных» трансаминаз в сыворотке крови в 3 раза превышает верхнюю границу нормы, следует прекратить прием препарата или уменьшить принимаемую дозу. Частота выраженных нарушений функции печени (связанных, в основном, с повышением активности «печеночных» трансаминаз), повышается при приеме 40 мг препарата. У пациентов с вторичной гиперхолестеринемией вследствие гипотиреоза, нефротического синдрома терапия основного заболевания должна проводиться до начала лечения розувастатином.
В ходе фармакокинетических исследований выявлено увеличение системной концентрации розувастатина среди пациентов монголоидной расы по сравнению с данными, полученными среди представителей европеоидной расы.
Одновременный прием розувастатина с ингибиторами ВИЧ протеаз не рекомендован.
При применении некоторых статинов, особенно в течение длительного периода времени, сообщалось о единичных случаях интерстициального заболевания легких. Проявлениями заболевания могут являться одышка, непродуктивный кашель и ухудшение общего самочувствия (слабость, снижение веса тела и лихорадка). При подозрении на интерстициальное заболевание легких следует прекратить терапию статинами.
У пациентов с концентрацией глюкозы от 5,6 до 6,9 ммоль/л терапия препаратом ассоциировалась с повышенным риском развития сахарного диабета 2-го типа.
Необходимо соблюдать осторожность при управлении автотранспортными средствами, занятиях потенциально опасными видами деятельности, требующими повышенной концентрации внимания и быстроты психомоторных реакций (риск развития головокружения).

Dosing and Administration

До начала терапии препаратом Сувардио® пациент должен начать соблюдать стандартную гипохолестеринемическую диету и продолжать соблюдать ее в течение всего периода терапии.
Дозу препарата Сувардио® подбирают индивидуально с учетом целевых показателей концентрации холестерина и индивидуального терапевтического ответа на проводимую терапию.
Рекомендуемая начальная доза препарата Сувардио® составляет 5 мг или 10 мг 1 раз в сутки как для пациентов, ранее не принимавших статины, так и для пациентов, переведенных на прием данного препарата после терапии другими ингибиторами ГМГ-КоА редуктазы.
При выборе начальной дозы следует руководствоваться концентарцией холестерина и возможным риском развития сердечно-сосудистых осложнений у данного пациента, а также следует оценить потенциальный риск развития побочных эффектов.
При необходимости через 4 недели можно скорректировать дозу препарата.
В связи с возможным развитием побочных эффектов при приеме дозы 40 мг по сравнению с более низкими дозами препарата, окончательное титрование до максимальной дозы 40 мг следует проводить только у пациентов с тяжелой формой гиперхолестеринемии и высоким риском возникновения сердечно-сосудистых осложнений (особенно у пациентов с наследственной гиперхолестеринемией), у которых при приеме дозы в 20 мг не была достигнута целевая концентрация холестерина, и которые будут находиться под врачебным наблюдением. Применение препарата Сувардио® в любых дозах противопоказано пациентам с тяжелой почечной недостаточностью (КК менее 30 мл/мин) (см. раздел «Противопоказания»).
Пациентам с почечной недостаточностью умеренной степени тяжести противопоказано назначение препарата в дозе 40 мг (см. раздел «Противопоказания»).
Patients with hepatic insufficiency
Повышение системной концентрации розувастатина у пациентов с баллом по шкале Чайлд-Пью, равным 7 и ниже, не выявлено. Однако повышение системной концентрации розувастатина наблюдалось у пациентов с баллами по шкале Чайлд- Пью 8 и 9. У таких пациентов следует контролировать функцию печени на фоне терапии розувастатином. Данные о приеме розувастатина пациентами с баллом по шкале Чайлд-Пью выше 9 отсутствуют. Пациентам с заболеваниями печени в активной фазе розувастатин противопоказан (см. раздел «Противопоказания»).

ethnic groups
У пациентов монголоидной расы возможно повышение системной концентрации розувастатина в плазме крови. Рекомендуемая начальная доза препарата Сувардио® у пациентов монголоидной расы составляет 5 мг. Применение препарата в дозе 40 мг таким пациентам противопоказано (см. раздел «Противопоказания»).
Пациенты, предрасположенные к развитию миопатии
Рекомендуемая начальная доза препарата Сувардио® для пациентов с предрасположенностью к развитию миопатии составляет 5 мг. Применение препарата в дозе 40 мг таким пациентам противопоказано (см. раздел «Противопоказания»).
genetic polymorphism
У носителей генотипов SLCO1B1 (ОАТР1В1) C.521CC и ABCG2 (BCRP) C.421AA отмечалось увеличение экспозиции (AUC) розувастатина в 1,6 и 2,4 раза, соответственно, по сравнению с носителями генотипов SLCO1B1c.521TT и ABCG2 c.421AA. Для носителей генотипов с.521СС или c.421AA рекомендуемая максимальная доза препарата Сувардио® составляет 20 мг один раз в сутки (см. Раздел «Фармакокинетика»).
concomitant therapy
Розувастатин связывается с различными транспортными белками (в частности, с OATP1B1 и BCRP). При одновременном применении препарата Сувардио® с лекарственными препаратами (такими как циклоспорин, некоторые ингибиторы протеазы вируса иммунодефицита человека (ВИЧ), включая комбинацию ритонавира с атазанавиром, лопинавиром и/или типранавиром), повышающими концентрацию розувастатина в плазме крови за счет взаимодействия с транспортными белками, может повышаться риск развития миопатии (включая рабдомиолиз) (см. раздел «Взаимодействия с другими лекарственными препаратами»; необходимо ознакомиться с инструкцией по применению этих препаратов перед назначением препарата Сувардио®). В таких случаях следует оценить возможность применения альтернативной терапии или временного прекращения приема препарата Сувардио®. При необходимости применения указанных выше препаратов следует оценить соотношение пользы и риска сопутствующей терапии препаратом Сувардио® и рассмотреть возможность снижения его дозы.
