Interaction with other drugs
When applied topically, the ointment Momat-C was observed the presence of very small, but measurable amounts of salicylate in blood plasma. According to published data on interaction with other substances salicylates after ingestion, or application to the skin may change action following therapeutic compounds: tolbutamide (butamida), methotrexate, heparin, pyrazinamide, agents promoting excretion of uric acid, drugs and anticoagulants. Other glucocorticosteroids and ammonium sulfate can affect the content of salicylic acid. These reactions should be borne in mind when assigning ointments Momat-C together with these compounds.
Overdose
Prolonged use of topically applied corticosteroids in excessive quantities can lead to the suppression of the hypothalamic-pituitary-adrenal system and to the secondary adrenal insufficiency. If the suppression of the hypothalamic-pituitary-adrenal system, it is necessary to stop the use of ointments or reduce the frequency of applications.
In case of toxicity caused by the salicylic acid, the use of ointments Momat-C should be discontinued. O salicylates to stimulate excretion of liquid to be administered. As necessary, appoint oral or intravenous administration of sodium bicarbonate, and potassium salts.
pharmachologic effect
Pharmacological group:
Glucocorticosteroid + topical keratolytic agent.
Pharmacodynamics:
The pharmacodynamic properties of the ointment Momat-C are directly connected with the presence of active components, mometasone furoate and salicylic acid.
Mometasone furoate is a potent glyukokorgikosteroidom. Like other topical corticosteroids, mometasone furoate has antiinflammatory, antipruritic, and vasoconstrictive properties. Glucocorticosteroids selection induces proteins that inhibit phospholipase A2 and collectively known as lipocortin. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor arachidonic acid. Arachidonic acid is released from membrane phospholipids by the action of phospholipase A2.
In experiments in vitro it was shown that mometasone furoate – a potent inhibitor of the synthesis of the three cytokines involved in the origin of the inflammatory process and its maintenance: interleukin-1 (IL-1), interleukin-6 (IL-6) and alpha-Tumor Necrosis Factor (TNF-?).
Salicylic acid causes exfoliation of the stratum corneum and, at the same time, does not result in qualitative or quantitative changes in the structure of viable epidermis. This mechanism of action to explain the dissolution of the intercellular adhesive.
Pharmacokinetics:
Suction.
After a single topical application of an ointment containing mometasone furoate (0.1%) in combination with salicylic acid (5%) for 12 hours without occlusive dressing to damaged skin, systemic uptake was found about 1.5% applied dose. The average content of salicylic acid in the plasma peak was 0.0908 mg / dL. Systemic toxic reactions to the presence of salicylic acid is usually associated with a significantly higher plasma levels (30-40 mg / dl).
Metabolism.
Absorb mometasone furoate is rapidly metabolized to produce different metabolites, compounds which are more polar than the starting material. Metabolites not possess pharmacological activity. The major metabolites of salicylic acid, quantified in plasma after topical ointment Momat-C – is gentisic acid (Cmax = 0,00127 mg / dL after 24 h) and salitsilurovaya acid (Cmax = 0.0171 mg / dL after 3.3 h ).
Withdrawal.
Derived primarily by the kidneys, and in a small amount in the bile.
Conditions of supply of pharmacies

side effects
Overall ointment Momat-C when applied twice daily is well tolerated. The most common side effects are mild to moderate burning sensation at the site of application of the ointment, itching, peeling.
Among the reported side effects with local corticosteroids on large areas of the skin following can be mentioned irritation, skin maceration, skin dryness, folliculitis, acne, hypopigmentation, perioral dermatitis, allergic contact dermatitis, occurrence of striae, sudamen, secondary infections, hypertrichosis . Children may suppress the function of the hypothalamic-pituitary-adrenal system with the development of Cushing’s syndrome.
special instructions
As with all drugs glucocorticosteroid must take the appropriate precautions in the event of an ointment on large areas of the skin or the intended long-term use. This warning is particularly important in pediatrics. Due to the fact that the ratio of skin surface and body weight in children is higher than in adults, children may be more sensitive to the action of glucocorticoids than adults, with the appearance of their symptoms suppression of the hypothalamic-pituitary-adrenal system and the manifestation of Cushing’s syndrome.
Ointment should not be used with occlusive dressings, including diapers and diapers. The drug is not intended for use on the face or in the groin and armpits. Ointment should not be used in ophthalmology. Children should receive the minimum amount of corticosteroids needed to achieve the effect. Long-term therapy with the use of corticosteroids may have undesirable effects on the growth and development of children.
In case of irritation, including excessive dryness of the skin and allergic reactions should stop using ointments Momat C and appropriate treatment.
If concomitant skin infection develops, it should be necessary to use an anti-fungal or anti-bacterial agent. If a positive response to treatment can not be achieved quickly, applying ointment Momat-C should be interrupted until such time as the infection is not properly resolved. It is necessary to evaluate the risk and the expected effect of the ointment Momat-C in case of the following diseases: diabetes, injury or inflammation of the peripheral blood vessels, irritation or infection of the skin. Steroids can cause changes in the appearance of the affected skin, making it difficult to correct diagnosis and leads to a slowing recovery.
