Не прерывайте лечение препаратом Локрен® резко и не меняйте рекомендованную дозу без предварительной консультации с врачом, так как это может привести к временному ухудшению деятельности сердца. Лечение не следует прерывать внезапно, особенно у пациентов с ИБС: внезапная отмена может привести к тяжелым нарушениям сердечного ритма, инфаркту миокарда или остановке сердца. Атриовентрикулярная блокада I степени: учитывая отрицательный дромотропный эффект бета-адреноблокаторов, при блокаде 1 степени препарат следует применять с осторожностью.
Стенокардия Принцметала: бета-адреноблокаторы могут увеличивать частоту и продолжительность приступов у пациентов со стенокардией Принцметала. Применение кардиоселективных бета1адреноблокаторов возможно при легкой степени тяжести стенокардии Принцметала или стенокардии смешанного типа при условии, что лечение проводится в сочетании с вазодилататорами.

Dosing and Administration
Препарат принимается внутрь и запивается достаточным количеством жидкости. Не следует разжевывать таблетку. Начальная доза препарата Локрен® для обоих показаний к применению препарата составляет обычно 10 мг (1/2 таблетки 20 мг). Если в течение 7-14 дней лечения не достигаются целевые значения артериального давления, то доза удваивается до 20 мг в сутки. Обычно не используются дозы препарата Локрен®, превышающие 20 мг (в связи с тем, что при увеличении дозы более 20 мг не наблюдается статистически достоверного увеличения антигипертензивного эффекта препарата). Максимальная суточная доза препарата Локрен® – 40 мг.
Пациенты с почечной недостаточностью: рекомендуется коррекция дозы в зависимости от функционального состояния почек пациента. При клиренсе креатинина более 20 мл/мин коррекции дозы не требуется. Однако, в начале лечения, рекомендуется проводить клиническое наблюдение до достижения равновесной концентрации препарата в крови, что достигается в среднем к 4-7-го дню лечения). При выраженной почечной недостаточности (клиренс креатинина менее 20 мл/мин) рекомендуемая начальная доза препарата составляет 5 мг/сутки (вне зависимости от частоты и дней проведения процедуры гемодиализа у пациентов, находящихся на гемодиализе), которая при недостаточной эффективности может увеличиваться в 2 раза каждые 1-2 недели. Максимальная суточная доза составляет 20 мг
Пациенты с печеночной недостаточностью: у пациентов с печеночной недостаточностью коррекции дозы обычно не требуется. Однако, в начале терапии, рекомендуется более тщательное клиническое наблюдение за пациентом.
