500 mg plus 400 IU
Indications
Prevention and comprehensive treatment of osteoporosis and its complications (fractures).
Prevention and treatment of calcium deficiency and / or vitamin D3.
Interaction with other drugs
vitamin D3 activity may decrease when it is applied simultaneously with phenytoin or barbiturates.
Hypercalcemia may potentiate toxic effects of cardiac glycosides while the use of calcium and vitamin D. electrocardiogram is necessary to control and the calcium content in serum.
Calcium can reduce the absorption of tetracyclines from the gastrointestinal tract. Therefore, drugs of tetracycline should take no less than 2 hours before or 4-6 hours after administration Complivit® Calcium D3 Forte.
To prevent absorption of drugs bisphosphonates decrease their recommended to take at least one hour before a Complivit® Calcium D3 Forte.
Glucocorticosteroids reduce calcium absorption, so glucocorticosteroid may require increasing doses Complivit® Calcium D3 Forte.
Simultaneous treatment with drugs cholestyramine or laxatives based on mineral or vegetable oil can reduce the absorption of vitamin D3.
With simultaneous use of thiazide diuretics series increases the risk of hypercalcemia, since they increase the tubular reabsorption of calcium. With simultaneous use of thiazide diuretics should be regular monitoring of serum calcium. Furosemide and other “loop” diuretics, on the contrary, increase the excretion of calcium by the kidneys.
Calcium reduces the effectiveness of levothyroxine decrease its absorption. The period of time between doses of levothyroxine and Complivit® Calcium Forte D3 should be at least 4 hours.
Suction quinolone antibiotics group, while the application is reduced with calcium supplementation. Therefore, quinolone antibiotics group should be taken 2 hours before or 6 hours after ingestion Complivit® Calcium D3 Forte.
Acceptance of food containing oxalate (sorrel, rhubarb, spinach) and phytin (cereals), reduces the absorption of calcium and should not be taken Complivit® Calcium D3 Forte within 2 hours after eating the sorrel, rhubarb, spinach and cereals.
Overdose
Overdose symptoms (hypercalcemia): thirst, polyuria, anorexia, nausea, vomiting, constipation, abdominal pain, bone pain, muscle weakness, fatigue, mental disorders, dizziness, headache, fainting, coma, nephrocalcinosis, urolithiasis, in severe cases – cardiac arrhythmias. Prolonged use of excessive doses (over 2500 mg calcium) – kidney damage, calcification of soft tissues.
In the case of detecting signs of overdose must stop receiving calcium and vitamin D, as well as the thiazide diuretics, and cardiac glycosides, and to consult a doctor.
Treatment: gastric lavage, replenishment of fluid loss, the use of “loop” diuretics (e.g., furosemide), glucocorticoids, calcitonin, bisphosphonates.
It is necessary to control the content of electrolytes in the blood plasma, kidney function and urine output.
In severe cases, the necessary measurement of central venous pressure (CVP) monitoring and electrocardiogram (ECG).
pharmachologic effect
Pharmacological group:
Calcium-phosphorus metabolism regulator.
Pharmacodynamics:
A combined preparation, which regulates the exchange of calcium and phosphorus in the body (bones, teeth, nails, hair, muscles). Decreases resorption (resorption) and increases bone density, filling the lack of calcium and vitamin D3 in the body, increases calcium absorption in the intestine and the reabsorption of phosphate in the kidney, promotes the mineralization of bone and tooth tissue.
Calcium – participates in formation of bone tissue, in maintaining a stable heart activity in regulation of nerve transmission, muscle contraction, hormone and is a component of the blood coagulation system.
Adequate calcium intake is particularly important in a period of growth, pregnancy and breastfeeding.
Vitamin D3 (Kolekaltsiferol) – increases calcium absorption in the intestine, and promotes the formation of mineralized bone and tooth tissue.
The use of calcium and vitamin D3 prevents the increase in the production of parathyroid hormone (PTH), which is a stimulator of increased bone resorption (leaching calcium from the bones).
Pharmacokinetics:
Calcium
Absorption: calcium absorbed in ionised form in the proximal part of the small intestine by active, D-vitamin dependent transport mechanism. Typically, the amount of calcium sucked into the gastrointestinal tract is about 30% of the dose.
Distribution and metabolism: 99% of the calcium in the body is concentrated in the rigid structure of bones and teeth. The remaining 1% is in the intra- and extracellular fluids. About 50% of the total calcium content in the blood is in the physiologically active ionised form, of which about 10% in combination with citrate, phosphate, or other anions, the remaining 40% bound to protein, primarily albumin.
Excretion: calcium derived intestines, the kidneys and sweat glands. Renal excretion depends on glomerular filtration and tubular reabsorption of calcium.
vitamin D3
Absorption: Vitamin D3 is readily absorbed in the small intestine (about 80% of the dose).
Distribution and metabolism: Kolekaltsiferol and its metabolites circulate in the blood associated with a specific globulin. Kolekaltsiferol converted in the liver by hydroxylation to 25-gidroksikaltsiferola. Then converted in the kidneys to the active form 1,25-gidroksikaltsiferol. 1.25 gidroksikaltsiferol metabolite is responsible for increasing calcium absorption. Unmetabolized vitamin D3 is deposited in fat and muscle tissue.
Excretion: Vitamin D3 is derived intestines and kidneys.
Pregnancy and breast-feeding
Complivit® Calcium D3 Forte used during pregnancy to replenish the calcium and vitamin D3 deficiency in the body. During pregnancy, the daily dose should not exceed 1500 mg calcium and 600 ME vitamin D3 (not more than 1 tablet Complivit® Calcium D3 Forte per day). Hypercalcemia on the background of an overdose during pregnancy can have an adverse effect on the developing fetus.
Complivit® Calcium D3 Forte is used during breast-feeding. Calcium and vitamin D3 can penetrate into breast milk, so you need to take into account the intake of calcium and vitamin D from other sources in the mother and child.

side effects
The frequency of side effects of the drug is seen as follows: very common:> 1/10, common:> 1/100,
special instructions
When long-term therapy should be monitored calcium and creatinine in serum. The observation is especially important in elderly patients with simultaneous treatment with diuretics and cardiac glycosides (see. See “The interaction with other drugs and food”), and in patients with an increased tendency to form kidney stones. In case of hypercalcaemia or signs of impaired renal function should reduce the dose or stop treatment.
Vitamin D3 should be used with caution in patients with renal insufficiency. In this case it is necessary to control the content of calcium and phosphate in the serum. It is also necessary to take into account the risk of soft tissue calcification.
To avoid overdose should take into account the additional intake of vitamin D from other sources.
Older people need for calcium is 1500 mg / day of vitamin D3 – 0,5-1 thousand IU / day..
Calcium and vitamin D3 should be used with caution in immobilized patients with osteoporosis due to the risk of hypercalcemia.
The simultaneous use of antibiotics tetracycline or quinolone is generally not recommended or should be done with caution (see. See “The interaction with other drugs and food”).
Effect on ability to drive and use machines:
The drug Complivit® Calcium D3 Forte does not affect the ability to drive or operate technically complex mechanisms.
Storage conditions
At a temperature of not higher than 25 C.
Keep out of the reach of children.
Dosing and Administration
Complivit® Calcium D3 Forte Chewable Tablets are taken orally during meal time. Tablets should be chewed or sucked. After this, you can drink water when necessary.
Adults: for the treatment of osteoporosis – 1 tablet 2-3 times daily, for the prevention of osteoporosis – 1 tablet 2 times a day 1 or 2 tablets once a day.
To make up for deficiency of calcium and / or vitamin D3: • Adults and children over 12 years – 2 tablets a day. • Children from 3 years to 12 years – 1 tablet per day or prescribed by a doctor.
Special patient groups
Patients with hepatic impairment:
No dose adjustment is required.
Patients with impaired renal function:
You should not use in patients with severe renal insufficiency.
Elderly patients:
The dose is the same as for adults. It should take into account a possible reduction in creatinine clearance.
duration of treatment
When used in prevention and in the treatment of osteoporosis, the duration of treatment is determined individually by the physician.
When applied to compensate for deficiency of calcium and / or vitamin D3 average duration of treatment not less than 4-6 weeks. The number of repeated courses is determined individually during the year.
