Contraindications

Interaction with other drugs
Drugs causing prolongation of the QT interval

В данной ситуации противопоказано применение препаратов, подавляющих перистальтику кишечника.
Гепатобилиарная система
При применении ципрофлоксацина отмечались случаи некроза печени и жизнеугрожающей печеночной недостаточности. При наличии следующих признаков заболевания печени, таких как анорексия, желтуха, темная моча, зуд, болезненный живот – прием ципрофлоксацина следует прекратить (см. раздел «Побочное действие»).
У пациентов, принимающих ципрофлоксацин и перенесших заболевание печени, может наблюдаться временное повышение активности «печеночных» трансаминаз и щелочной фосфатазы или холестатическая желтуха.

Режим дозирования у пациентов пожилого возраста (после 65 лет)
Пациентам пожилого возраста следует назначать более низкие дозы ципрофлоксацина в зависимости от тяжести заболевания и показателя клиренса креатинина.
Дети и подростки
Легочная форма сибирской язвы (постконтактное воздействие) 2×15 мг/кг массы тела (максимальная доза 500 мг)
Режим дозирования при легочной форме сибирской язвы (лечение и профилактика)
Прием препарата следует начинать сразу после предполагаемого или подтвержденного инфицирования.
Общая продолжительность приема ципрофлоксацина при легочной форме сибирской язвы составляет 60 дней.
Режим дозирования при нарушениях функции почек или печени у взрослых
1. Клиренс креатинина (мл/мин/1,73 м2) – от 30 до 60
Креатинин плазмы(мг/100 мл) – от 1,4 до 1,9
Максимальная суточная доза ципрофлоксацина при пероральном приеме максимально 1000 мг
2. Клиренс креатинина (мл/мин/1,73 м2) – ниже 30
Креатинин плазмы(мг/100 мл) – >2,0
Максимальная суточная доза ципрофлоксацина при пероральном приеме максимально 500 мг
Пациенты с почечной недостаточностью на гемодиализе
1. При клиренсе креатинина от 30 до 60 мл/мин / 1,73 м2
(умеренная почечная недостаточность) или его концентрации в плазме крови от 1,4 до 1,9
мг/100 мл максимальная пероральная доза ципрофлоксацина должна составлять 1000 мг в сутки.
2. При клиренсе креатинина 30 мл/мин / 1,73 м2
и менее (тяжелая почечная недостаточность) или его концентрации в плазме крови от 2 мг /100 мл или более, максимальная пероральная доза ципрофлоксацина должна составлять 500 мг в сутки. В дни проведения гемодиализа ципрофлоксацин принимают после осуществления процедуры.
Амбулаторные пациенты с почечной недостаточностью, находящиеся на непрерывном перитонеальном диализе
Максимальная суточная доза ципрофлоксацина должна составлять 500 мг (1 таблетка препарата по 500 мг).

Пациенты с почечной и печеночной недостаточностью
Режим дозирования аналогичен описанному в пунктах 1 и 2.
Дети с почечной недостаточностью и/или нарушениями функции печени
Режим дозирования у детей с нарушениями функций почек и печени изучен не был.
Продолжительность терапии
Продолжительность лечения зависит от тяжести заболевания, клинического и бактериологического контроля. Важно продолжать лечение систематически, не менее 3 дней после исчезновения лихорадки или других клинических симптомов.
Средняя продолжительность лечения:
– 1 день при острой неосложненной гонорее и цистите;
– до 7 дней при инфекциях почек, мочевыводящих путей, органов брюшной полости;
– весь период нейтропении у пациентов с ослабленным иммунитетом;
– от 7 до 14 дней при других инфекциях.
При инфекциях, вызванных Chlamydia spp., лечение следует продолжать не менее 10 дней.
